Test Results and Sickness Certificates
Sick notes
For work abscences of seven days or less you need to obtain a Self-Certificate Form SC2 from your employer.
Doctors are not required to, and do not issue, NHS sick notes for the first seven days of illness.

If you require a certificate for the first seven days from your doctor, please ask, in writing, for a private sick note for which a fee is payable. See our guide to Non NHS Fees, including private sick notes here. A receipt will be issued for this.
If you are unable to return to work after seven days, contact the surgery to obtain an NHS sick note.
You can use Rapid Health or contact us via e-mail: hiowicb-hsi.parksurgery-admin@nhs.net for this type of enquiry. Please include as much information as possible (reason for abscence, sickness start date, expected sickness end date) and ensure we have your current contact details - incase we need to contact you to discuss the request further.
Blood and other test results - requested by GP team
Due to the large volume of test results that come into the surgery, we are unable to contact patients about normal or expected test results and we do not routinely contact patients to discuss results that are not clinically relevent.
Please, whenever possible, use the NHS App to view your results instead of phoning reception - this really helps us manage the team workload, and keeps the phones and front desk less busy for when you really need us.
The surgery only receives tests results for investigations requested by the surgery team - we do not receive results generated by hospital teams - unless they specifically write to us with the result.
On receiving results directly from the laboratory or radiology department, the requesting GP will "file" the result in your notes, at which point it will become visible to you on your NHS App.
If you can see a particular result on your NHS App, then you know that the GP has both seen and interpreted it, accounting for your individual clinical context.
If you wish to discuss the results, then please contact the surgery via Rapid Health, to arrange a GP appointment.
If you decide to phone the surgery to find out a test result, please remember that receptionists cannot interpret them, and will only be able to read the result to you as it is appears in the notes, together with any comments that the GP may have made. Please note that the GP is unlikely to comment on every result, particularly ones that are not clinically significant or relevent.
Where clinically necessary, the Doctor will contact patients to discuss any unexpected or abnormal results to establish whether any further action is required.
Blood and other test results - requested by Hospital teams
Where a test has been requested by the hospital consultant or clinic, please contact them directly to discuss the result.
We do not receive results of investigations arranged by the hospital team. The requesting hospital team receive results directly as soon as they are available, and bear the clinical responsibility for their timely interpretation, communicating the results to the patient, as well as initiating of any further necessary actions.
Please do not ask the surgery to chase up these hospital initiated test results on your behalf, you should liase directly either with the Specialist team, the Consultant Secretary or the Hospital "PALS" (patient advice liason service) who will chase this up on your behalf.
